We are hiring a dedicated عامل الدعم الشخصي (PSW) لـ community health travel role في كينغستون، أونتاريو. In this role, you’ll provide home-based care to individuals and families in the community, focusing on health promotion, disease prevention, and chronic disease management.

This opportunity is ideal for PSWs who are passionate about community care and supporting individuals to live independently and healthily in their own homes.

• Assist clients with personal hygiene, grooming, dressing, and mobility

• Provide support with daily activities such as meal preparation and medication reminders

• Perform routine health checks and assist with chronic disease management

• Offer companionship and emotional support to clients and families

• Maintain accurate documentation of services provided

• Ensure safe and clean environments for clients in their homes

المتطلبات

• سارية المفعول PSW certification or equivalent Health Care Assistant credential

• الحد الأدنى 6 months of experience in home care or community health

• الحالي الإنعاش القلبي الرئوي القلبي الرئوي/الإسعافات الأولية التصديق

• Clear background check and up-to-date immunization record

• Excellent communication and interpersonal skills

• سارية المفعول driver’s license and access to a vehicle (preferred)
