We are hiring a dedicated Assistant(e) personnel(le) (PSW) pour un community health travel role en Kingston, ON. In this role, you’ll provide home-based care to individuals and families in the community, focusing on health promotion, disease prevention, and chronic disease management.

This opportunity is ideal for PSWs who are passionate about community care and supporting individuals to live independently and healthily in their own homes.

Principales responsabilités

• Assist clients with personal hygiene, grooming, dressing, and mobility

• Provide support with daily activities such as meal preparation and medication reminders

• Perform routine health checks and assist with chronic disease management

• Offer companionship and emotional support to clients and families

• Maintain accurate documentation of services provided

• Ensure safe and clean environments for clients in their homes

Exigences

• Valable PSW certification or equivalent Health Care Assistant credential

• Minimum 6 months of experience in home care or community health

• Actuel RCP/premiers secours certification

• Clear background check and up-to-date immunization record

• Excellent communication and interpersonal skills

• Valable driver’s license and access to a vehicle (preferred)
