Key Responsibilities

• Conduct home visits and community outreach assessments

• Provide wound care, education, and medication support

Requirements

• RN license with NANB

• Community or home care experience preferred

• Valid driver’s license and reliable transportation

• Strong independence and accountability

• Able to handle a varied caseload
