Job Overview

We are seeking a dedicated and autonomous Registered Practical Nurse (RPN) for a travel home care assignment in Petrolia, Ontario. In this role, you will deliver high-quality nursing services to clients in their homes, supporting wellness, independence, and chronic disease management.

Ideal for RPNs who enjoy flexible, one-on-one patient care and thrive in community-focused settings.

Key Responsibilities

• Provide in-home care including medication administration, wound care, and vitals monitoring

• Support clients with chronic conditions, post-discharge care, and wellness education

• Complete accurate charting using mobile EMR systems

• Coordinate with interdisciplinary care teams and family members

• Travel to multiple client residences within assigned region

• Promote safety and independence in the home environment

Requirements

• Active RPN license with the College of Nurses of Ontario (CNO)

• Minimum 1 year of home care, community nursing, or LTC experience

• Valid driver’s license and reliable vehicle

• Current CPR/BLS certification

• Clear background check and immunization record

• Self-motivated with strong communication and time-management skills
